Part 2 of the Pulmonary Arterial Hypertension (PAH) Education Series.

Presentations will feature case-based discussions and include opportunities for interactivity to emphasize the clinically relevant and practical points.

  2. Coordinate effectively with PH centers and multidisciplinary teams to optimize care transitions and shared decision-making for patients diagnosed with PAH
  3. Participate in shared monitoring of individual treatment plans, manage therapy response, and address common comorbidities and complications in collaboration with PH centers
  4. Navigate advanced care pathways when therapies fail, including referral for transplant, palliative approaches, and end-of-life care planning
  5. Integrate emerging therapies and future innovations into long-term care planning, patient education, and follow-up
